Dear Patients:
I am leaving my practice on May 31, 2024 due to a family emergency. I will continue to provide care to patients until this date.
I am in the process of finding a family doctor in order to continue to provide care at our clinic after I leave. I will update you when and if I am able to find someone.
If you prefer, you may contact your medical plan/health insurance company for a list of area physicians. You may also contact the local medical society or the physician referral service at a hospital of your choice for names of physicians near you.

You would need to fill out a “Release of Medical Records” form, which could be found at www.drthangnguyen.com/forms under “Form C” and attach it to your email request.

ABOUT DR. THANG NGUYEN
Dr. Nguyen has been practicing for over eighteen years. He is a thorough, detail-oriented primary care and family physician treating both adult and pediatric patients. His approach to treatment emphasizes patient education about disease processes and general health and well being. He holds certification from the American Board of Family Medicine.
Dr. Nguyen obtained his Bachelor's of Science Degree in Biochemistry and Molecular Biology from the University of Wisconsin, Madison. After his undergraduate program, Dr. Nguyen pursued his medical degree from the University of Wisconsin, Madison, School of Medicine and graduated with his Doctorate of Medicine. He continued his medical education at the University of California, San Francisco where he received his residency training in the Family and Community Medicine Program.
Patients visiting Dr. Nguyen can receive a full range of medical services. He provides annual physicals, general medical consultations, as well as treatment for specific illnesses and issues.
